Social Services

Henry J. Austin Health Center is pleased to offer its patients the services of a Certified Social Worker and a Licensed Clinical Social Worker.

Austin Center understands that sometimes life can throw a person a curve. Social work services are available at Austin Center to assist patients in responding to life’s concerns and emergencies, so that being healthy can remain a priority.

Social work services include, but are not limited to:

• Assistance in applying for help with the costs of medications
   
• Information and referral regarding community housing options
   
• Assessment regarding substance abuse services
   
• Family support and intervention for safety
   
• Assistance in accessing community programs for food and nutrition
   
• Information regarding any of Austin’s programs, including nutrition services, SWEET (childhood obesity services), translation, transportation, Sharing the Care, and others
   
• Goal-setting for healthy lifestyle changes

Social workers at Austin Center work closely with the staff of Austin’s Behavioral Health program, which provides assessment and treatment for a variety of mental and behavioral health concerns.
